What Is FUE Hair Transplant and How Does It Work?

FUE (Follicular Unit Extraction) is a modern hair transplant technique in which hair follicles are extracted one by one using micro punch tools and transplanted to bald or thinning areas. Introduced by Dr. Rassman in 2003, the technique quickly surpassed the older FUT (strip) method and has become the most preferred approach worldwide. The fundamental principle of FUE is to extract DHT-resistant follicles from the back and sides of the scalp and transplant them to the recipient area, where they continue to grow permanently — protected from genetic hair loss.

The donor area is typically the back of the head and the sides above the ears. Hair in this region is genetically resistant to androgen hormones, so when transplanted, it does not fall out and continues to grow lifelong in its new location. The procedure is performed under local anesthesia, and the patient remains conscious without feeling pain.

FUE Hair Transplant Procedure: Step by Step

1. Consultation and Planning: Hair analysis is performed, donor capacity is measured, the target area is calculated, and a new hairline is designed with the patient. The pattern of hair loss is determined according to the Norwood Scale.

2. Preparation and Anesthesia: The donor area is shaved, cleansed with antiseptic solution, and local anesthesia is applied. Some clinics use needle-free anesthesia (jet injector) to make this stage more comfortable.

3. Graft Harvesting: Each follicle is individually extracted using micromotor or manual punch tools (0.6-0.9 mm diameter). Harvested grafts are stored in special HypoThermosol or Ringer's Lactate solution at +4°C to preserve follicle viability — a critical step for graft survival.

4. Channel Opening: Micro channels (0.6-1.0 mm) are opened in the recipient area using slit blades or sapphire blades. The angle of the channels (15-25 degrees) must match the natural hair growth angle; otherwise, the result will not look natural.

5. Graft Implantation: Follicles are placed into the channels using forceps or implanter pens. Single-hair grafts are used for the front hairline, while 2-3 hair grafts are used in the rear areas to achieve natural density. The entire procedure takes 6-9 hours.

Who Is a Suitable Candidate for FUE?

Certain criteria must be met for a successful FUE hair transplant. Suitable candidates: Men experiencing Norwood Scale 2-6 hair loss, with sufficient donor capacity, stabilized hair loss, good general health, age 25+; and women with diagnosed genetic alopecia.

Unsuitable candidates: Young men with active, unstabilized hair loss (typically under 22), individuals with insufficient donor capacity, patients with uncontrolled diabetes or bleeding disorders, and oncology patients undergoing chemotherapy.

Recovery Process After FUE: Stage by Stage

Days 1-3: Mild swelling, redness and itching are normal during the first three days. The first wash is performed at the clinic, and the patient is instructed on home washing protocol. Alcohol, smoking and strenuous physical activities should be avoided.

Days 7-14: Crusts begin to fall off. Hair is not yet visible, but follicles are settling into their new location.

Weeks 3-5 (Shock Loss): This is the most worrying phase for patients. Most of the transplanted hair sheds. This is completely normal — the follicles remain, the hair shafts will regrow.

Months 3-6: New hair starts to grow thin and sparse. You're at about 30-40% of the final result.

Months 6-12: Hair thickens and becomes denser. You reach 70-80% of the final result.

Months 12-18: The final result is visible. Hair reaches full density and natural appearance is complete.

How Many Grafts Can Be Transplanted in One Session?

An average of 2,000-4,000 grafts can be transplanted in a single session. In what are called "mega sessions," 5,000-6,000 grafts can be performed. However, the number of grafts varies for each patient and depends on factors including donor capacity, size of the bald area, hair type (straight, wavy, curly), and clinical standards (graft extraction speed). The key consideration in mega sessions is to avoid over-harvesting the donor area.

Advantages and Disadvantages of FUE

Advantages: No linear scarring; only tiny dot scars in the donor area; rapid recovery (return to daily life in 2-3 days); low complication rate; performed under local anesthesia; permanent and natural-looking results.

Disadvantages: Lengthy procedure time (6-9 hours); requires an experienced specialized team; slightly higher cost than FUT; donor area limitations when very large numbers of grafts are needed.

FUE Hair Transplant Cost in Turkey

FUE hair transplant cost at quality clinics in Turkey ranges from 1,500-3,500 Euro. This price typically includes 5-star hotel accommodation, airport transfers, medications and special shampoos. The same procedure costs 8,000-15,000 Euro in Europe and the United States. Important: avoid extremely cheap offers, choose clinics that offer real surgeon expertise, and verify certifications. Hair transplantation is a one-time investment; focusing on quality rather than the lowest price is much wiser in the long run.
