What Is Rhinoplasty and Why Is It Performed?

Rhinoplasty, commonly known as a "nose job," is a surgical procedure performed to improve the size, shape or function of the nose. It is one of the most common aesthetic surgeries worldwide and can be performed for both cosmetic concerns and medical reasons such as breathing difficulty, deviated septum, post-traumatic deformities, or congenital anomalies. In 25-30% of cosmetic rhinoplasties, functional issues are simultaneously corrected.

Open vs Closed Rhinoplasty: Which Is Right for You?

Rhinoplasty can be performed using two main techniques:

Closed Rhinoplasty: All incisions are made inside the nose, leaving no visible scars. Recovery is shorter and swelling is less. The surgeon's working space is limited, so this technique is preferred for simpler corrections.

Open Rhinoplasty: A small incision is made on the columella (the skin bridge between the nostrils), and the nasal skin is lifted to fully visualize the internal structure. It's preferred for complex deformities, revision cases and detailed aesthetic corrections. The scar fades over time and becomes nearly invisible.

The surgeon decides which technique is best after consultation. Modern rhinoplasty also uses newer techniques such as "preservation rhinoplasty" and "ultrasonic rhinoplasty."

Ultrasonic Rhinoplasty: A New Generation Approach

Ultrasonic rhinoplasty uses piezo devices powered by ultrasonic waves instead of traditional bone saws. These devices only cut bone tissue, sparing surrounding soft tissues, blood vessels and nerves. As a result, post-operative swelling decreases by 50% and bruising by 70%; recovery time is significantly shorter.

How Is Rhinoplasty Performed?

Rhinoplasty is typically performed under general anesthesia and takes 1.5 to 3.5 hours. The steps are:

  1. Anesthesia: General anesthesia is administered.
  2. Incision: Open or closed technique is chosen.
  3. Bone and Cartilage Reshaping: The bone hump on the nasal bridge is filed down, and the cartilages at the tip are reshaped.
  4. Septum Correction: The deviated nasal septum is straightened.
  5. Closure: Dissolvable sutures close the incisions, and small internal tampons are placed.
  6. Splint Application: A plaster or thermoplastic splint is placed on the nose for 7 days.

Post-Operative Recovery Process

First week: Splint and internal tampons in place. Mild swelling, bruising under the eyes, and nasal discharge are normal. Tampons are removed after 2-3 days, and the splint after 7 days.

First 2-3 weeks: 60-70% of swelling subsides. Most patients return to social life.

First 3 months: 85-90% of swelling resolves. The nasal tip may still be slightly swollen.

6-12 months: The final shape emerges. Residual swelling at the tip resolves completely.

For the first 4-6 weeks after surgery:

  • No glasses (contact lenses can be used)
  • Nasal cleansing with prescribed solution
  • Avoid intense exercise and heavy lifting
  • Sleep with head elevated on high pillows
  • Protect from direct sunlight
